Some help? Please?

Ok so my website design and information is all there! Now for the part i have no clue on how to do it.

My website is so people can subscibe and i have the pay pal buttons for this. But this button is just to allow the payment, i wont get any information such as name, second name, email which are things usauly on a form, and the most important bit is the email.

How do i get a form that has name: email: and so on and also something for pay pal?

They have paypal button and fully customizable form layout.
The catch? Like most form generators they charge a monthly fee for their service.

@Rex, what are you using to build your site? Do you just need something where users can submit information like name, email, etc.? If you're using Wordpress to build your site, they will have plugins to do that. Even if you use other frameworks, they will have it too.

There should be lots of free forms out there, im pretty sure that Paypal even offer forms which they allow you to use where you can collect this information. ive made many of them and usually based them off forms ive found online. Check Paypal website because im sure that they have them.
